ATHLETIC FORMS and DRAGONFLY

For Middle and High School Students
To ensure safety during all sports participation, every student-athlete must complete and pass a sports physical before beginning athletic practices and/or competitions. This physical form will last one year from the time the physical is complete. In addition, EVERY year guardians and athletes are required to sign required forms such as concussion awareness, cardiac arrest awareness, and the heat and humidity policy. To meet the required standards, all forms are completed using the DragonFly Max platform.

Pre-Participation Physical Exam:  The GIAA requires a pre-participation physical exam for each student who participates in middle and high school sports. This exam should be completed by a medical doctor of the athlete’s choice. Physical exams are valid for ONE calendar year from the date they were issued. Download the four-page form (History, Physical Examination & Medical Eligibility) and once completed by a medical doctor, upload the entire form to Dragonfly. GIAA Physical Form

Required Athletic Forms:  GIAA requires each school to keep several signed documents on file (Sudden Cardiac Arrest, Concussion Awareness, and Heat Awareness).  You will find these forms in Dragonfly.  Please make sure you electronically sign all three forms.

Required Copy of Insurance Card:  All athletes must have a copy of their insurance card on file.  Please upload a picture of the front and back of your insurance card to Dragonfly.

New to DragonFly Max?
To access DragonFly Max, parents and/or athletes should download the DragonFly Max app through their local app store OR visit the Dragonfly Max website from a desktop/laptop computer. When creating an account, it will ask for you to enter the name of the school or school code. Please enter Bulloch Academy or CAGYYF.
After creating an account, a request to join the BA community will be sent to an account administrator and will generally take one day to be processed.

How do I complete the required forms?
Once approved, guardians can fill out information such as the athlete’s medical history and insurance documentation. Completed physical forms can be scanned and uploaded to the athlete’s profile.
All other required forms can also be found on each athlete’s profile. For guardians with multiple athletes, only one account needs to be created. Each athlete will have their own profile (including required forms) under one account.
